HIV Surveillance Clinical Nurse Specialist.
NMC registered Nurses only
An exiting opportunity to make a real difference to patients living with HIV in the local areas covered by Frimley Health. This post holder will be the point of contact for any inpatient with a new or existing diagnosis of HIV. The post holder will support the opt out HIV testing initiative and will be pivotal in ensuring patients with HIV are referred to community HIV services on discharge form the acute hospital site. The post holder will provide a safety net to ensure any newly diagnosed patients with HIV are promptly identified and supported in Frimley Health and followed through to our community partners.
This is a fully cross site role, support with further HIV training will be provided.
To work as a clinical nurse specialist supporting inpatients within Frimley Health who have HIV, co-ordinating care, supporting clinical treatment and signpost to specialised advice, support, training and education for patients, heath and social care professionals and carers.
This post is cross site and will cover Wexham Park Hospital and Frimley Park Hospital Sites and any other Frimley Health site. There is also an expectation of close working with the Garden Clinic (Wexham Park Hospital and Solent NHS Trust and Central Northwest London NHS Foundation Trust (Frimley Park Hospital).
This post is part of the particular the home infection management service (HIMS) which is one part of the Intravenous and vascular access services team (IVAS). In addition to focusing on HIV there will be other opportunities to assist the HIMS and IVAS team with related clinical work as and when required.
To monitor and make contact with newly diagnoses HIV inpatients within Frimley Health.
To provide ongoing clinical signposting and guidance for all medical and nursing staff within Frimley Health relating to patients who are admitted to Frimley health with an existing diagnosis of HIV and where to access ongoing specialist clinical HIV services on discharge.
To be a point of information for staff and patients in all aspects of accessing community HIV services.

To signpost to specialist counselling service for inpatients, outpatients and their significant others.
To participate in the inpatient HIV multi-disciplinary team clinical and managerial meeting weekly.
To co-ordinate specialist HIV care training and education across all professional boundaries with Frimley Health .
To support investigations and provide information regarding complaints and clinical incidents in related to HIV care within Frimley Health supporting the lead nurse consultant.
To work with external organisations in relation to HIV
