A sensational opportunity has arisen to join the well-establish, innovative, and dynamic Respiratory Services. We are expanding our successful teams to provide an excellent quality of care to our patient of care, and we welcome an experienced and motivated Band 6 Clinical Nurse Specialist/Practitioner who is passionate about delivering excellent respiratory patient centred care. You will be working closely within a multidisciplinary team of staff including Respiratory Consultants, Physiotherapists and Clinical Nurse Specialist, Advanced Clinical Practitioners, HCAs and Therapy Support Workers. You will also interact with GPs, Urgent Care response Teams, Community Nurse and Palliative Care Specialists.
The successful candidate will play an integral part in improving the standard of care for respiratory patients and will have the opportunity to work within a variety of settings. The award-winning team is based at Farnham Community Hospital, but we see patients as hospital in-patients, in clinic and in their own homes. The focus is supporting people to manage their condition within the most appropriate setting.
The respiratory clinical nurse specialist (CNS/SP) is a skilled nurse who uses clinical communication and interpersonal skills to identify the needs of people with chronic respiratory conditions and oxygen users and gives care that recognises social, cultural and spiritual differences within their lifestyle. The CNS/SP has clinical knowledge in this specialist field and works independently and as part of a multidisciplinary team.
As part of the Respiratory team the CNS/SP will be responsible for the co-ordination and care of patients with respiratory illness to ensure they receive the highest quality care. You will be part of the specialist workforce and use advanced assessment skills and knowledge to review patients in their own home, clinics or in hospital and ensure they receive the best clinical outcomes. You will ensure that patients discharged from hospital following an admission with their chronic lung condition will have appropriate care and support to avoid readmission, advise and assess patients in the community for home oxygen therapy and respond promptly to community requests for admission prevention for patients suffering an exacerbation of their lung condition.
The job will require working flexible hours (including unsocial hours) and in other designated locations across NE Hampshire, Farnham and Surrey Heath as well as the primary base (Farnham Hospital). In particular, flexibility is required across secondary care and community.

Develop and maintain high standards of communication, including liaising with other health care professionals and clinical lead to ensure the safe and effective development of services to all respiratory patients on the caseload, but particularly to ensure the needs of Oxygen and COPD patients are met.
Working closely within a multidisciplinary team of staff including Respiratory Consultants, Physiotherapists and Clinical Nurse Specialists in primary and secondary care, Advanced Clinical Practitioners, HCAs and Therapy Support Workers. You will also interact with GPs, Urgent Care response Teams, Community Nurse, Palliative Care Specialists and other virtual wards.
Ensure that patients suffering from chronic lung conditions and those on oxygen therapy receive effective counselling /education. This often involves breaking bad news on diagnosis and prognosis
Visit patients suffering from chronic lung conditions in their homes to assess their condition, through the use of advanced assessment skills; plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care.
Manage own caseload of respiratory patients and familiarise with all aspects of the Respiratory Service and Virtual Ward SOP/Clinical Flowcharts and Escalation Procedures;
To provide oxygen assessment and review to those patients referred to the service at home or within the clinic setting as per guidelines, considering the most appropriate modality should oxygen be required and performing a risk assessment.
To maintain competence in using the blood gas and CRP analyser, and spirometry equipment.
Maintain awareness of current advances and changes in clinical practice and update training accordingly
