Description:
Develop and maintain high standards of communication, including liaising with other health care professionals and clinical lead to ensure the safe and effective development of services to all respiratory patients on the caseload, but particularly to ensure the needs of Oxygen and COPD patients are met. Working closely within a multidisciplinary team of staff including Respiratory Consultants, Physiotherapists and Clinical Nurse Specialists in primary and secondary care, Advanced Clinical Practitioners, HCAs and Therapy Support Workers. You will also interact with GPs, Urgent Care response Teams, Community Nurse, Palliative Care Specialists and other virtual wards. Ensure that patients suffering from chronic lung conditions and those on oxygen therapy receive effective counselling /education.
This often involves breaking bad news on diagnosis and prognosis Visit patients suffering from chronic lung conditions in their homes to assess their condition, through the use of advanced assessment skills; plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care. Manage own caseload of respiratory patients and familiarise with all aspects of the Respiratory Service and Virtual Ward SOP/Clinical Flowcharts and Escalation Procedures; To provide oxygen assessment and review to those patients referred to the service at home or within the clinic setting as per guidelines, considering the most appropriate modality should oxygen be required and performing a risk assessment. To maintain competence in using the blood gas and CRP analyser, and spirometry equipment. Maintain awareness of current advances and changes in clinical practice and update training accordingly
