This role is a new and exciting opportunity in the Trust for you to take your career to the next level.
RDC offer a GP urgent referral service for patients with non-specific symptoms which could indicate cancer, as well as additional site specific Two Week Wait pathways.
RDC provide a rapid evaluation of the patient to reach a definitive diagnosis, advice to the patient on management of their condition, signposting to appropriate support services and rapid onward treatment should cancer be diagnosed.
Be a point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice and psychosocial support.
Accept referrals to agreed criteria, and work in partnership with other professionals to deliver coordinated programmes of care effectively and referring patients to other practitioners as required.
Deliver expert nursing care to patients in agreed practice settings, working as part of nursing/MD teams.
Escalating when appropriate and deemed necessary within professional judgment.
Provide appropriate education to patients and their families to promote health and encourage self-care.
To make independent clinical decisions and initiate investigations, care/treatment and technological intervention within an agreed framework.
Following completion of the non-medical prescribing course and registration with the NMC, undertake non-medical prescribing in accordance with the Trust non-medical prescribing policy.
