Description:

Clinical & Operational: - Accept referrals via agreed protocols, inclusion and exclusion criteria within the service - To draw appropriately from a range of assessment tools and methodologies - Ensure that care is delivered with a clinically safe, cost-effective and evidenced based approach - Support and share the best practice in the development and delivery of virtual wards - Have the ability to operate in a highly political and sensitive environment, adopting a non - judgemental, but influential approach - Plan and prioritises own workload, exercising autonomous professional responsibility and attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate - Provide clinical supervision to all members of the multi-disciplinary team who are involved in the virtual ward pathways - Evaluate clinical competence of staff using a performance framework and standard tools of measurement - Complete all requirements relating to data collection within the service and keep coherent records of all clinical activity in line with Trust policies and guidelines - Carry out clinical audits of service line performance, including patient feedback and evaluations, and collate and disseminate the results for feedback Quality & Performance: - Take overall accountability for the development and maintenance of quality improvement outcomes for patients, ensuring that there is regular reporting in place and action plans for improvement, where necessary - Understanding of the importance of value for money and efficient use of resources in the design and implementation of pathways across the Division - Actively support the improved outcomes for patients by driving up the continued quality of care - Provide strong and consistent leadership and have the ability to work across boundaries, divisions and systems - Support and enable the delivery, review and evaluation of the virtual wards, and act upon areas for improvement - Actively lead on specific pieces of project work as defined by the Divisional Director of Operations Surgery - Make a significant ongoing contribution to programmes of research, audit and evaluation in both the local and national programmes for virtual wards - Adhere to the national guidelines and expectations around the virtual ward agenda. Act upon requests for information swiftly and professionally - Engage and develop sound relationships with patients, members of the public and key external stakeholders within the Voluntary Sector, to ensure that the best quality care and resources are available to our virtual ward patients - Have a sound understanding of health inequalities and how these can particularly be linked to the opportunities around virtual ward provision.
