Description:

Job Purpose The post holder will provide a specialist clinical pharmacy service to Mental Health and Learning Disability inpatients at Langdon Hospital (Secure Services). Where there are service needs, the post holder will also be expected to provide specialist clinical pharmacy services to other areas across the Trust. In doing so they will: Implement the Trust Medicines Optimisation (MO) strategy so that agreed, robust systems and processes are in place to ensure safe, high quality and cost-effective MO services. Ensure that these systems meet the requirements of the people who use our services, their relatives and carer, the Trust and the regulatory bodies.
Assist with the development, implementation, audit and evaluation of MO policies, standard operating procedures, clinical protocols, practice standards, safety briefings and prescribing guidelines. Provide expert advice on specialist pharmaceutical matters and manage risks associated with medication; including provision of information regarding medicines use, expenditure and place in treatment to promote cost effective, evidence based prescribing with support from the Senior Clinical Pharmacist and wider MO Team. Provide information and advice about medicines to patients and/or their carers to promote informed consent and ensure safe and effective use of medicine and minimise waste. Address and resolve interface issues with primary care and other organisations, to ensure accurate medicines reconciliation and smooth transfer between care environments.
Contribute to incident reporting and reviews. This post is open to early career pharmacists on a band 6 to 7 progression development basis. For the right candidate, the post can be for a Foundation Pharmacist (band 6) and therefore a training post suitable for entry level or newly qualified Pharmacists. There will be an expectation for the post holder to undertake personal development based on the HEE Foundation pharmacist curriculum, which includes completion of a postgraduate qualification in mental health pharmacy if this is not already held
