DESCRIPTION
The Clinical Pharmacist for Credena Health is responsible for coordination of medication related services as it pertains to specialty pharmacy. Responsibilities include promotion of evidence-based, safe, effective drug therapy, medication counseling, patient assessments, and standardization of medication practices across the continuum.

REQUIRED QUALIFICATIONS:
- Education to meet certification, license or registration requirement.
- Upon hire: Oregon Pharmacist License
- 2 years Minimum 2 years experience in retail, specialty, or ambulatory care pharmacy practice
