Description:
The post holder will work alongside the OSM, Team Leads and the Occupational Therapy Professional Lead toprovide specialist input to strategic planning, development, audit and evaluation of clinical services, protocols and policies within the specialised area.This post holder will lead and work with the multidisciplinary CARE Point Community Team and other teams within the Community Response Services to ensure that a high standard of care is delivered. They will work as part of a multidisciplinary team to provide efficient and effective facilitation of the patient journey through hospital and in community, to achieve safe and timely discharge, seamless transitions and help avoid unnecessary admission and re admission to hospital. The post holder will provide a comprehensive service in the specialised area and provide clinical expertise at an advanced level in the assessment, diagnosis and management of highly complex clients. The post holder will be expected to triage and allocate referrals to ensure patients are seen within an appropriate response time.
The post holder will be expected to offer support and supervision to staffwithin the team and ensuring NHCT policies and procedures are adhered to.There will be an expectation to promote and ensure safe working practice for all team members both within the office environment and in the wider community at all times, adhering to the lone working policy,health and safety policy, risk assessments,moving and handling assessments etc. There will also be occasions where the post holder will facilitate student placements within the team e.g. nurses, social workers, occupational therapists, trainee doctors etc. To role model compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the trust.
This means demonstrating a consistent leadership style which (a) engages, enables and empowers others (b) uses coaching to promote ownership of learning and quality improvement and (c) facilitates team working and collaboration within teams / departments and across organisational boundaries
