Description:

We are now recruiting for a CNM, Patient Flow and Care Transition for the newly opened Leopardstown Care Centre (Ballyogan Road, Dublin 18, D18 A3VR)

NOTE

Given the nature of the work, it must be stressed that maximum flexibility is required. This job description is not exhaustive and does not detail every activity. Specific duties, tasks and objectives will be agreed with the post holder periodically by the Director of Nursing in the interests of the service.

What are the qualifications, skills and experienced required?

  • 3 years post-registration experience in a relevant field and experience in managing patient caseload.
  • Hold a recognised Managerial Qualification (minimum level 6 on the National Framework of Qualifications).
  • Registered on the NMBI live register.
  • Clinical experience working in older persons care or nursing home care.
  • Working experience of HIQA Standards and Health Act Regulations 2013.
  • Fluent in English language (verbal and written) and be eligible to work in Ireland.
  • Strong communication and listening skills.
  • An understanding of the Assisted Decision-Making process
  • Ability to work well as part of a care team.
  • Caring and compassionate and be passionate about providing high quality person-centred care.
  • Have a basic knowledge of IT systems such as excel spreadsheets.
  • Have a good understanding of applications for home supports and NHSS/Fair deal

Responsibilities:

To work as part of the Patient Flow team with the ADON, Patient Flow Manager. To ensure the safe and effective management of patient referrals to Mount Carmel Community Hospital from the acute hospitals. To facilitate admission and follow through regarding discharge planning. To act as case manager for patients as required.

Duties and Responsibilities

  • Receive patient referrals from acute hospitals; review referrals and liaise with Discharge Coordinators.
  • Ensure that there is a discharge plan and anticipated date of discharge for all patients referred to Mount Carmel Community Hospital (MCCH).
  • Arrange for accepted referrals to be admitted to MCCH.
  • Act as case manager for patients as required and as agreed by Patient Flow.
  • Adhere to all Mowlam Healthcare policies and procedures.
  • Maintain clear, concise, and informative assessments and records within the hospital in accordance with legislation and Mowlam Healthcare standards.
  • Knowledge and awareness of HIQA standards (Safer, Better Healthcare).
  • Ensure all admissions to the hospital are carried out correctly and all appropriate documentation completed.
  • Attend ward handovers daily and weekly MDT meetings.
  • Provide cover for the wards if required as necessary.
  • Liaise with doctors about admissions, prescriptions and ensure admission paperwork is ready each day.
  • Follow-up on patient’s outpatient appointments if necessary.
  • Facilitate patient admissions/discharges.
  • Refer patients to Medical Social Worker as required.
  • Have an understanding of the Assisted Decision-Making process and act as a case manager for patients as required.
  • Ensure effective communication with all referral sources in the hospitals and discharge agencies in the community.
