Description:
The Community Continence Specialist Nurse is centred on improving the quality of life for individuals with bladder and bowel dysfunction. The expectation is the post holder will split their time between the Extended Primary Care Team (EPCT) 80% working with house bound patients and 20% with the outpatients Continence and Pelvic Rehabilitation team.
The post holder will work (80%) across the EPCT, enhancing community awareness, supporting staff with complex continence issues and assessments on bladder and bowel care. They will work proactively highlighting high use patients, working to improve care & patient experience, avoid 999 calls, Emergency Department attendance and admission avoidance.
You will manage a caseload of complex adult patients (over 18) in the community who require bladder and bowel assessment and care. This will involve domiciliary visits to housebound patients with complex needs.
The post holder will also work with Continence and Pelvic Rehabilitation team (20%), providing line management to a small number of resisted nurses including working clinically.
The post holder will provide specialist education, advice and support to Community Health Newham staff, service users and carers.
To monitor spend on containment products and ensure that any risk of overspend is highlighted and to advise on remedial measures to minimise overspend.
