Description:
An exciting opportunity for a qualified and highly motivated registered nurse to join the expanding Elderly virtual ward team to deliver frailty in reach into A+E and to support the development of a front door frailty same day assessment unit.
The successful post holder will work as a dynamic and proactive specialist nurse within our very successful and developing Integrated Frailty service. The service operates over 7 days and provides the frail population of Bradford the benefit of a holistic Comprehensive Geriatric Assessment.
You will be based at the front end of the hospital, working collaboratively to start the assessment process from the moment the patient arrives in the emergency department or our assessment wards, including the frailty same day emergency care unit.
As a team member you will be part of a friendly, supportive, and professional integrated team which comprises of Consultants , Geriatricians, Nurses, Occupational Therapists, Physiotherapist, Coordinators, Advanced Clinical Practitioners, Pharmacists, Social services and Community services.
We are looking for an experienced nurse who is highly motivated and a self-starter with the ability to work as part of a multidisciplinary team and provide skills, knowledge and expertise in the care and management of frail older patients in a community setting.
You will be expected to manage caseloads, allocate visits to staff and be a key member at multi-disciplinary team meeting.
It is essential that you have a full UK driving license and have a car available for work.
