As the Community Health Connection Coordinator, you will play a pivotal role in individuals’ lives, making a significant impact through:

  • Conduct outreach and engagement activities in greater capital region based on member lists provided by CDPHP
  • Facilitate referrals to non-medical resources such as case management, housing, and other providers as necessary
  • Work with key practices and providers to coordinate preventative care events
  • Conduct regular supervision with the CDPHP Wellness Advocate
  • Submit required activity documentation consistently by expected deadlines

Community Health Connection Coordinator Minimum Salary: $24.87
Community Health Connection Coordinator Maximum Salary: $24.87
Community Health Connection Coordinator Schedule: Monday - Friday

Qualifications:

  • 1-3 years of experience with a Bachelor’s degree
  • 3-5 years of experience with a High School Diploma/GED
  • Working knowledge of the social determinants of health
  • Experience working with a diverse population
  • Must have: personal cell phone, valid driver’s license, proof of current car insurance and registration, and access to a reliable and working vehicle
