Description:

15 hours across 7 days a week on a permanent contract.
The service operates between the hours of 9am – 5pm, 7 days per week, 365 days per year.
A rare and exciting opportunity has arisen for an experienced, enthusiastic and motivated Band 6 Community Health Nurse to join our hard-working Long Term Conditions team. The candidate will have to be able to prioritise their own workload without supervision and be able to work well on their own initiative as well as part of a team, they will need to be flexible, approachable and have a sound clinical evidence-based background and knowledge.
As part of the Long Term Conditions team, you will be implementing the House of Care framework, providing holistic assessments within a patient’s home.
The post holder will work autonomously as part of a highly motivated team of Case Managers providing a high standard of care and utilising holistic skills in the assessment, planning and implementing pro-active care needs to a population of patients with long term conditions in the community settings.
The post holder will work in collaboration with Primary and Secondary care clinicians, social care and mental health colleagues, to ensure care is patient centred and appropriate to their assessed needs.
