Description:
Steward Medical Group at Hawthorn Medical Associates seeks an enthusiastic, professional Community Health Worker with a helpful mindset, and the ability to work both independently and as part of a team in a fast-paced environment! The successful candidate will be responsible for:
- Assisting clients in their homes, community or clinic setting in accessing health related services. Educating clients/patients on the purposes of the program and the impact it may have on their well-being as well as provide information for alternative services.
- Facilitating communication and coordination of services between providers and the clients/patients. Coordinate and monitor services including tracking of compliance in relation to care plan objectives.
- Document all client encounters and contracts and maintain client files and notes.
- Travel to client homes, community locations, schools and other outreach destinations.
- Perform miscellaneous job related duties as assigned.
Qualifications:
- High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
- Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
