Description:
An exciting opportunity has arisen to work for the South Kensington & Chelsea Mental Health Hub (CMHH).
Based in Fulham Road, Chelsea, the South K & C CMHH is looking for a passionate Mental Health worker who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities.
There are two Community Mental Health Hubs in Kensington & Chelsea, one North and one in the South, each is aligned to a local Primary Care Networks (PCN). Working in this way allows each Hub to have a core structure and function to meet the needs of the local population, aligned with our primary care colleagues. There is good interaction between our hubs, fostering a supportive and friendly atmosphere of mutual support and trust.
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
The successful applicant will take the lead of a caseload of patients and be at the forefront of their care. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the Inpatient Services and Home Treatment Team.
As a case manager you will assess health and social care needs and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.
You will proactively undertake and ensure rigorous risk assessment & risk management strategies are applied, helping to identify and mitigate risks as well as encourage individual growth and development for your patients through informed positive risk taking
You will apply specialist clinical knowledge and skills to carry out evidence-based interventions, which may be individual or in groups, which are underpinned by relevant theoretical approaches and in line with the Trust’s Research and Development Strategy; to lead and supervise junior staff in this practice as required.

Flexible working - Our Hub operates Monday to Friday from 9.00 am to 5.00 pm however there are opportunities for this post to be tailored to suit you and your lifestyle needs with some flexible working available.

To be accountable for the care and support of a defined caseload of adults of working age.
To assess health and social care needs (including accommodation, finances, and support in accessing education, training or employment) and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty and undertake assessments of need (in compliance with the Care Act 2014)
To undertake risk assessments and develop robust risk management plans, according to Trust policy
To use agreed assessment and care planning tools such as DIALOG
To safely administer and store medication as needed in the community and ensure that relevant Pharmacy guidelines and policies are adhered to.
