Description:

We are recruiting a registered nurse to join our professional team of district nurses who care for people in their own homes. This includes care homes and occasionally nursing home input. We pride ourselves in caring for people at home and subsequently reducing hospital admissions and unnecessary disruption to patients and their families.
District nurses deliver many aspects of care to people in their own homes. This includes diabetic management, all aspects of wound care, palliative care, venepuncture, catheter management, holistic nursing assessment of needs, medication administration. All care is assessed using the Welsh Levels of Care to assess acuity. District Nurses work collaboratively with other MDT’s and closely with GP Practices in the local area.
The district nurses work autonomously but are an integral part of a team environment. Visits are made to patients in their own homes on a one to one basis and sometimes as a joint visit.
Support and training will be given for anyone new to the community and team support is available at all times.
There is an abundance of job satisfaction in caring for people in their own homes with the ability to offer continuity of care and getting to know the extended family support network.
