Description:

An exciting opportunity has arisen at Luton Community Mental Health Hub for Band 6 Community Mental Health Nurses and Learning Disabilities Nurses to be allocated professional at the front door to offer a welcome call to service user triage and offer intervention from the Hubs. Completing initial assessment and screen for risk more effectively whilst attending triage
Our teams core hours are Monday - Friday 9am - 5pm and operate a Duty Professional system responding to those who require support in a crisis. Our team provides assessment, care planning, intervention and evaluation of those with a moderate to severe mental health diagnosis for ages between 18 to 65 (up to 70 if already open to the team). Our service focuses on developing and utilising your specialist mental health skills and embracing your core competencies as a Registered Nurse. Working within our team you will be involved in the planning of new services.

Responsibilities:

Attending MDT triage
Making Contact with the service user in a timely Manner following from Triage to screen for risk effectively and to complete initial assessment.
To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
Ensuring that the values outlined in the NHS Constitution are adhered to daily and any matters of concern are raised with the relevant Line Manager or through the necessary processes within the Trust. Responsible for delivering a compassionate, dignified and respectful service to
patients at all times.
Continually delivering and maintaining high standards for a defined caseload as agreed with Line Manager.
Leadership, supervision and support to junior staff.
Ensuring national targets and standards are achieved. Building trust and developing relationships to ensure service user involvement.
Demonstrating a professional manner in all work carried out.
Maintaining accurate and timely records.

The team is made up of a rich resource of multidisciplinary professionals who support psychological ways of working. The team includes Registered Nurses, Social Workers. Doctors, Psychologists, Assistant Psychologist, Occupational Therapists, Support Recovery Workers, Psychotherapists, Administrators and an Employment specialist as well as working closely within a designated locality with Primary care mental health link workers, the Local Authorities and voluntary services. Our team has recently seen developments with its restructure of new experienced Consultant Psychiatrists who have brought with them a wealth of specialist knowledge and experiences.

To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to.
To establish and maintain good relationships with service users, carers and voluntary sector colleagues.
To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.
Liaise with General Practice and Primary Care colleagues to promote partnership working.
