Description:
Would you like to get out and see beautiful views of the Coast and Moors whilst carrying out your work as a Community Nurse?
We are looking for Community Nurses to work in our Frailty Virtual Ward Service. Working across, Scarborough, Whitby and Ryedale localities, delivering care in patients home setting/usual place of residence, in line with the VW frailty pathways and national strategy. There would be an excellent induction, support from Senior Nursing Staff & a Preceptorship Programme for newly qualified nurses.
The service operates 7 days a week.
Ideally applicants would have at least 6 months post registration experience and an interest in community nursing, frailty and ageing well. As a community nurse, you would be supported to develop into an autonomous practitioner; nursing patients with multiple complex health care needs in their own home.
As a Community Nurse, you will work within an integrated hub model of Community Nurses, Specialist Nurses, Community Staff Nurses, Physiotherapists, Occupational Therapists, Registered Nursing Associates, Assistant Practitioners & Health Care Assistants to meet the needs of the complex patient in the community and avoid admissions to the acute sector as far as practicable. This includes (once competencies have been achieved):
Intravenous Antibiotic administration;
End of life care as appropriate to service model
Recognising and Managing the deteriorating patient;
Complex and simple wound care
Phlebotomy, sample collecting, swabs in line with process
Point of care monitoring, remote monitoring digital platforms
Utilising physical assessment skills and clinical nursing skills to complete patient assessments including falls, nutrition, pressure ulcers and the deteriorating patient.
