Description:

This is an exciting opportunity for a Registered Practitioner to become a Community Practice Educator. The post holder will need experience and confidence in driving learning within the Integrated Community Care Group.
The successful candidate will work closely with all community but will be predominantly linked to providing education and support for the 2-hours response, virtual wards, induction, preceptorship, community clinical skills and band 3 development

The post holder will work alongside with the Quality and Service Improvement Matrons, Project Support Officers and Clinical Service Managers to support and facilitate:

  • Core Skills Training
  • Induction
  • Preceptorship
  • Patient Safety Training
  • One-to-One Support

The post holder will need to work flexibly and agilely across the Integrated Community Care Group to be able to engage all clinical teams.
The post holder will be responsible for ensuring the provision of high quality multi professional learning environments within the Integrated Community Care Group.
This will require the post holder to work locally within services where inter professional learning opportunities can be promoted.
You will be joining an enthusiastic and motivated team who very patient safety focused. The Community Practice Educator Role will identify opportunities and lead on the delivery and review of inter professional learning opportunities.
As part of the Quality and Service Improvement Team the aim is to develop all staff to the highest standard and promote a culture of patient safety.
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
