Description:

A Monday-Friday 8am-4pm working pattern is just one of the attractions this opportunity provides. We are looking to recruit a Band 6 Community Respiratory Nurse based at Coley Clinic in Reading and covering the Berkshire West locality.
You will be a qualified, registered nurse and will have experience in respiratory medicine and the supervision of junior colleagues and students. We are looking for someone who will lead on raising awareness in chronic obstructive pulmonary disease (COPD) management and we will give you the opportunity to develop upon existing knowledge and skills alongside developing an understanding of service needs and organisational strategies from a management perspective.
As part of the Community Respiratory Team you will use clinical reasoning skills, clinical research, audit and data collection to provide support and advice in a community setting.
A real people person, you will be working with the Community Based Respiratory Team as part of the Cardiac and Respiratory Specialist Services (CARSS) including Pulmonary Rehabilitation, the Heart Function Service and Cardiac Rehabilitation, all based at Coley Clinic. Your energy and drive will be rewarded with the support of a dedicated team that strives to create a progressive, friendly and considerate environment.
1. Work as part of an integrated team leading the care for those with chronic respiratory disease.
2. Work as part of a 2 Hour Rapid Response Service with the aim of reducing the need for acute service intervention – reducing admissions, supporting early discharges, supporting colleagues and empowering patients to manage their condition with the ultimate aim of improving quality of care.
3. Work within an integrated service to improve the lives of patients with respiratory conditions and heart failure.
4. Work as part of the Home Oxygen Assessment and Review Service (HOS-AR) to assess patients in Berkshire West who have an oxygen prescription, ensuring that this is appropriate for the patient’s needs and requirements.
5. Work as part of the team delivering Pulmonary Rehabilitation courses.
