Description:
Aspects of the role include To assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patients diverse needs, their carers and other professionals Deliver interventions that are evidenced based Be responsible for the safe monitoring, management and administration of medication Work autonomously, building leadership skills and advanced clinical assessment skills You will build effective relationships with patients, their carers, family and colleagues across the MDT You will develop exceptional planning and organisational skills You will be able to analyse care and treatment provided, and develop sound clinical decision making You will be trained to deliver high quality care and develop Knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, understand bladder and bowel dysfunctions, provide supportive care to carers and family members You will delegate and support junior staff, building leadership skills A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays
