Lincoln Older Adults CMHT is offering a rare and exciting opportunity to join the OA CMHT based at Witham Court who can compliment the current CMHT team who provides assessment, and treatment of the OA of all ages who have Mental health difficulties that are experiencing memory problems in the community with their day to day functioning in Lincoln area.
Applicants are invited from a highly motivated and dynamic individuals with a positive attitude to deliver the trust Visions and Values in their day to day practice providing the best healthcare experience for people under our care through evidence based practice to individuals using our services, using effective communication and promotes a culture of transparency and equity. You will be part of a supportive and caring team providing clear pathways to our patient group.
The role will involve working alongside OA CMHT colleagues to follow the individual care plans, medication management, social care aspects to support patients to achieve their potential whatever their diagnosis is so they can live well in the community and moving them towards discharge with the right support and network to meet their needs.

You will be required to go to patients place of residence to follow care plans, take patients to appointments/reviews, psychological interventions, advocate for them in a variety of circumstances with GP’s, Councils, Social Services and other external agencies ensuring their potential in their activities of daily living is reached. You will manage your own diary to enable you to work flexibly and responsively to your caseload of service users.
The post holder will be adaptable to the role whilst supporting the Multi Disciplinary Team and the Quality Agenda of the Older Adults Division. Demonstrate/possess excellent IT skills to ensure that able to manage diary/commitments, complete accurate case recording and complete daily tasks. Demonstrate good time management and organisational skills, and an ability to plan and prioritise workload.

To work within the policies, protocols and clinical procedures of Lincolnshire Partnership NHS Foundation Trust and to promote competent working practice within an integrated multi-disciplinary team.
To liaise with the team and wider Multi-disciplinary Team to carry out assigned duties across the locality.
To manage a caseload of clients and service users with a diagnosis of Dementia, mental health and or their support network, including family, carers and significant others.
Following assessment of need to signpost referrals to the most appropriate service whether internal or external or both to the organisation.
To be prepared to work on own initiative as necessary and report to the team and locality manager.
Provide detailed and timely personal support plans for patients and carers where necessary
Provide a timely response and review (KPI) for all referrals to the service
Promote equality for all patients and understand their health needs.
Demonstrate an awareness of service user and carer perspectives on the provision of treatment and continuing care. Identifying their unmet needs and signposting appropriately.
Ensure accurate record keeping in accordance with Trust policy.
Participate regularly in clinical and managerial supervision sessions.
Liaise with representatives from associated voluntary and statutory organisations and facilitate good communication processes.
Attend in-service training and other study / refresher days to meet individual, statutory and service needs.
Participate in performance reviews.
To work flexibly across a 5-day-a-week service
