Description:

Team Leader To manage the dietetic community team and support performance management through annual appraisal and interim review. To promote personal and professional development and to keep them abreast of changes within the organisation. Ensure the team receive appropriate training and development and attend mandatory/statutory training in line with Surrey Downs policy To lead the dietetic community team to review best practice and challenge current practice To lead on projects to improve the efficiency of the team and service. To implement and monitor outcome measures to demonstrate quality of the clinical work and have support processes to ensure the sustainability of the quality To monitor community caseloads and activity and ensure safe workloads To participate in staff recruitment, participate in interview panels and inducting new members of staff To represent the community dietetics service at meeting as required To collaborate and work closely with the other team leaders to ensure consistent delivery of clinical standards across Surrey Downs Community and Home enteral tube feeding patients To coordinate and lead the provision of dietetic services for SDHC area To communicate complex, scientific nutritional information to patients and carers with barriers to communication and understanding from a multicultural background To provide a domiciliary service to clients identified as high priority if required To contribute to the delivery of group education programmes to patients, carers, medical staff and other health care professionals within the SDHC area To attend and contribute to multi-disciplinary meetings, and to keep abreast of latest development in HETF and general dietetics To provide support to primary health professionals including GPs, nurses and medical students, to ensure accurate and consistent dietary advice (which is in line with national service Framework and NICE guidelines) is given to all patients on a range of nutritional issues To provide a domiciliary service to identified home enterally fed patients, liaising with Nutricia Homeward Enteral Feed Nurses and Enteral Feed Assistants and other enteral feed companies as appropriate, and ensuring accurate assessment, advice, training and dietary intervention To participate in the development of standards and protocols for Home Enteral Tube Feeding, ensuring these guidelines are evidence based and conform to clinical governance requirements.
To monitor and review these standards / protocols and advise the clinical manager of the future need
