Description:

An exciting new opportunity for a one-year fixed term/secondment has arisen for 3x Band 7 Complex Care Co-ordinators. The posts are for motivated individuals who have an interest and passion to work with Autistic people who have complex needs admitted hospital or other specialist inpatients
The opportunity is within the Review and Resettlement Team based at Heathside Maidstone. The Review and Resettlement is a team of clinical professionals who work with patients placed in out of area speciality placements who monitor their care and facilitate transition back to Kent and Medway.
This role allows you the flexibility of maintaining a good work-life balance with an option of working remotely.
You will gain experience of working with patients with complex needs, the CTR process, work closely with external providers such as the Integrated Care Board (ICB) , Social Services Kent and Forensic to facilitate timely discharges from hospital.
You will be required to have a least 2 years experience working at Band 6 with good communication skills, able to demonstrate a flexible approach to your work, self-motivated, innovative, professional and a team player.

The post holder will support the Review, repatriation and where possible discharge of Kent, Medway and out-of-area hospitals autistic patients to either services or homes closer to or in the Kent & Medway region. They will contribute to the implementation of operational, clinical, and interdisciplinary strategies to ensure the delivery of a high standard of service and in achieving national and Trust objectives, and will contribute to the overall corporate objectives of the organisation through upholding Trust values.
The post holder will work as a member of the Review and Resettlement Team (RRT) that will sit within the current Patient Flow service and be actively involved in the discharge planning process of autistic patients to improve the patient journey, minimise patient delays and ensure a timely review process is upheld. They will be responsible for the coordination of complex discharges as well as those requiring minimal intervention to facilitate a timely discharge of patients following hospital admission.
To ensure that the Review and Resettlement service is consistent across Kent and Medway and will work in collaboration with colleagues from the ICB and CSU placement teams, ICB commissioners, KCC and Medway Council, external providers and senior managers within KMPT.
