Description:
This post is part of a planned expansion and development of Older Peoples’ Services in Dudley Group of Hospitals. The post holder will be covering frail elderly surgical patients in the surgical wards and Frailty Assessment Unit.
The postholder will be performing 3 weekly rounds to review surgical patients pre/post operatively and provide ad-hoc review of urgent and unwell patients. This post has been covered by the locum consultants for last 8 years and mortality rates have improved significantly in these patients.
The post holder will also provide 4 sessions in Frailty Assessment Unit which is part of Same day Emergency cover (SDEC). We get patients from ED, AMU, community and HOT clinic follow ups. We have also started virtual ward which is helping admission avoidance in frail elderly patients.

Responsibilities:
Review of frail elderly patients in the surgical wards
Review of Frail patients in Frailty Assessment Unit.
To develop and train the clinical team to deliver this service.
To measure and monitor this new service by audit and other means.
To provide prospective cover to other colleagues in the department.
