Description:

Bradford Teaching Hospitals NHS Foundation Trust is looking for proactive, motivated, forward thinking and dynamic individuals to join our award winning Care of the Elderly clinical team.
We are offering the successful applicant the opportunity to work closely with the existing elderly care physicians to provide a high standard of care across the Trust. We are seeking consultants with enthusiasm, commitment and drive to work closely with colleagues throughout the organisation and within the community to help lead the organisation.
The elderly care department includes 3 acute wards, 4 community hospitals and a virtual ward. The department treats about 7,000 acute patients per year and has excellent links into other relevant departments, both within the foundation trust and externally. Intermediate care services including Community Hospitals and Virtual Ward, providing opportunities to develop a more effective interface between acute and community based services for our older population with a discharge to access model.
The post holder will work with the existing team of consultants to deliver the Elderly Care service across the trust, with particular focus on Interface geriatrics and developing community based services with a focus on admission and readmission avoidance.
The post holder will be expected to support Virtual Ward that has been established as an enabling multidisciplinary team (nursing, medical, therapy and social care) to support older people at home. The team provides a supported discharge to patients from acute wards via “step down” model of discharge to assess or an ambulatory care pathway. The virtual ward also supports admission avoidance by providing the same multidisciplinary team to patients “stepped up” from community or accident and emergency department. Moving forward we are working towards embedding a ‘comprehensive geriatric assessment’ in all patients, with a view to preventing admissions from primary care.

The post holder will participate in a 1 in 9 acute on call rota , category A , covering the Elderly Care wards (wards 3, 29, 31 F5 and F6), and responding to community based calls as required.
The post-holder will be expected to contribute to the usual clinical activities of the department which will include outpatient clinics, MDT attendance and participating in the ward Consultant rota. The consultant will provide continuing responsibility for patients in their clinics and in partnership with colleagues.
The post holder will develop services aimed at avoiding readmissions to hospital, through active follow up of high risk patients on discharge from hospital to avoid repeated acute admissions where possible.
In addition the post holder would be expected to take on significant responsibility in the supporting activities for the department within areas of clinical governance taking into account any areas of interest and consultant colleagues’ current commitments. This includes participation in the department’s service development, clinical governance, MDT review, clinical trials, mortality and business meetings.
The department is linked to the Academic Unit of Elderly Care and Rehabilitation, established with joint funding by the foundation trust and Bradford University. Our research mixes methods approaches to investigate service outcomes for older people, including patients with stroke, in both secondary (hospital) and primary care. Our current funding includes Medical Research Council (MRC) clinical trials grant and National Institute for Health Research (NIHR) programme grant.
