Description:

The Enfield Crisis Resolution and Home Treatment Team is an innovative and creative multi-disciplinary team, always looking to improve the service it provides to its community.
The team is looking for a talented Band 4 Graduate/Associate Mental Health Worker who will be responsible for assessing a mental health crisis and delivering safe and effective home treatment for people in accordance with their care plan and risk assessment under supervision of senior clinicians within the service
The right candidate should be forward thinking, able to work on their own as well as part of the team as the role will involve being responsible for risk assessment and management, care planning and care delivery within a team approach. The post holder should be confident in assessing and managing mental health and social needs and be able to deliver a range of therapeutic techniques. Devising care plans in partnership with the people that are referred, and their carers/families is essential. The post holder will work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients’ mental health and overall functioning, ensuring needs are met.

1. May be asked from time to time to undertake a range of duties which relate to direct patient care, under supervision following competency assessment
2. Assist in the coordination of care delivery for a delegated number of service users under the supervision of a registered professional, in close collaboration with the Directorate, psychiatric multidisciplinary team, service users and other community agencies.
3. Work under the supervision of qualified staff that supervise their caseload.
4. Contribute to the monitoring of the physical environment ensuring that it is safe and conducive to the therapeutic work of the unit/community setting
5. Have informed knowledge and understanding in the psychological, social,
physical and spiritual needs of all service users
6. Actively participate in the care and support of service users as per the plan of care
7. Maintain clear, timely, comprehensive and contemporaneous written clinical records on all patients in accordance with Trust policies
8. Utilising IT skills and knowledge to access and input clinical information and data to the Trust Electronic System (RiO) and Local Authority IT system.
