Description:
POSITION SUMMARY
This position is responsible for providing the mobile and office-based Crisis Intervention services for the Walworth County Department of Health and Human Services (WCDHHS) including answering the emergency line, assessing for & coordinating hospitalizations, assisting with scheduling intake appointments, monitoring court orders, and coordination & linkage services. This position routinely assesses and coordinates care for consumers experiencing mental health and substance abuse crises.

EDUCATION AND/OR EXPERIENCE
Bachelor’s degree in social work or a related field and seven months to one year of relevant prior experience. Proven experience in ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals as well as ability to compute rate, ratio and percent and to draw and interpret bar graphs.
CERTIFICATES, LICENSES, REGISTRATIONS AND OTHER SPECIAL REQUIREMENTS
Valid Wisconsin driver’s license.
Proof of minimum liability auto insurance coverage.
Responsibilities:
This job description reflects management’s assignment of essential functions; it does not prescribe or restrict the tasks that may be assigned.
Assess consumers experiencing mental health emergencies. Collaborate with law enforcement to develop safety plans or coordinate involuntary hospitalizations in accordance with Chapter 51.
Assist law enforcement with Protective Custody placements and coordinate safety plans or arrange admission to detoxification facilities.
Provide information and referrals to match consumer needs.
Provide crisis intervention services in multiple settings, including on the phone, in consumer homes, in the community or at WCDHHS offices. Services may be provided in coordination with other community partners.
Provide transportation for consumers during crisis situations.
Complete Adult-at-Risk and Elder Abuse/Neglect reports. Interview individuals reporting alleged maltreatment.
Document contacts with consumers in various databases.
Contact appropriate on-call personnel for after-hours emergencies.
Provide case management services including assessment, crisis planning and coordination and linkage to community services.
Demonstrate a commitment to county safety and risk management efforts.
