Description:

Data Management and Digital Transformation: Use data and improve data quality to understand service demand, capacity and activity and to drive improvements in patient access, operational efficiency, and staff experience Identify digital technology that can improve service delivery and patient outcomes and optimise the use of digital tools within service delivery and performance monitoring/ management Work with the operations and/or network manager to formulate and present business cases for the purchase of and/or use of this technology Support the adoption of new technology and other initiatives to enhance service delivery and patient access to care Enable PCN and, where appropriate, practice staff, to work more effectively to support the sustainability of general practice services via the promotion of PCN services and the NHS App, the use of cloud telephony, digital triage and online consultations Collate data to monitor service performance against indicators as agreed Work with the Practice manager, to support development of initiatives to improve effectiveness, efficiency and quality of operational processes to ensure the smooth running of the practice and services Support the Practice to develop appropriate IT understanding and skills within the teams and train new staff as required Support the effective adoption of national and local data and technology-related initiatives as agreed by the practice Responsible for annual submission of DPST and any actions to aid this Responsible for website and social media channels Support the corporate operation of the Practice, working with the Practice Manager and Partners to ensure meetings are effectively managed, agreed actions minuted, logged and delivered QOF: Produce QOF reports via SystmOne and Ardens Manager to support monthly QOF meetings. Highlight focus areas of QOF to ensure that targets are met. Identify coding errors impacting QOF achievement. Provide guidance and support to the clinical team on correct coding.
Undertake work such as invites and communication for patient groups. Co-ordinate invite reports for LTCs Enhanced Services: Monthly reporting of data quality for all LES and DES work. Quarterly reporting and submission via CQRS Local and CQRS National portals. Quarterly submission of LES and DES activity.
