Description:

To support the delivery of the programme of work as part of one of the system wide work streams for the Lincolnshire population, managing this from planning to implementation and benefits realisation.
The post holder will support a significant change programme in transforming dementia and neurodiversity services across the system in line with the national and local strategic ambitions.

The specific purpose of this role will be to support the Head of Mental Health Transformation, and all System Partners, to deliver the Dementia and Neurodiversity Transformation programme of work; specifically: -

  • Dementia Strategy and Action Plan
  • ADHD pathway
  • Supplementary Neurodevelopment pathways including but not exclusively Functional neurodevelopment disorder (FND) TICS and Tourette’s
  • Autism strategy and pathways

Support the development, leadership and oversight of the delivery of the Lincolnshire dementia and neurodevelopmental transformation programmes.
Contribute to, and lead as appropriate, monitoring the progress of all projects within the programmes of work and ensure the development of mitigations where these are off track.
Support clinical, operational, and corporate services across a range of health, social care and VCFSE organisations in the identification, planning and implementation of service redesign and transformation opportunities to improve quality of care.
Lead and manage any staff allocated to the area of work where appropriate.
Participate in identifying data, and analysis requirements and co-producing the design for data gathering and desired outputs.
Participate in the planning and engagement with clinicians and stakeholders to understand the case for change for the development of future models of care and contribute to the implementation of change.
Participate in the effective commissioning of services to deliver the objectives of the work programme, including financial planning and objectives.
Attend the Lincolnshire relevant meetings and participate in all other key meetings, acting as Chair when appropriate.
Systematically act upon findings in the LACE ADHD pathway review and improve the care provided by all services to provide better quality services.

We are required to reduce expenditure to sustainable levels (in alignment to the ICS 5-year strategic plan) which broadly fall into three types of scheme: system wide transformation, system wide efficiency, ICB only efficiency.
We are looking to a recruit a dynamic, patient centred Dementia and Neurodiversity Transformation Programme Manager to work for the Lincolnshire ICB.
It is an exciting time to join us and help shape our response to the key areas of Dementia and Neurodiversity, supporting the delivery of our Autism, Dementia strategies and Neurodiversity pathways. This role is to support the Head of MH Transformation responsible for oversight of Strategic transformation functions of the ICB supporting the delivery of key national indicators and you will be able to shape delivery of both ICB and provider delivered services.
This role is to progress the development and delivery of strategy and pathway transformation across the Integrated Care System.
You will need to be an experienced programme manager with a track record of delivering and supporting transformation across complex systems for people with Mental Health, Dementia and/or Neurodiversity conditions and have an in-depth knowledge of commissioning acquired through post graduate diploma or equivalent experience or training, plus further specialist knowledge or experience to master’s level equivalent
