  • Demonstrated skills in teaching, mentoring and coaching others
  • Experience in Australian residential aged care and or community aged care
  • High level of negotiation, communication and relationship building skills
  • Excellent problem solving and decision-making skills
  • Ability to prioritise multiple tasks and meet deadlines
  • Excellent written and verbal communication skills
  • Proficient in Microsoft Office Suite with excellent administration skills
  • A current Australian drivers licence and willingness to travel
  • Empathy and patience
  • Relevant qualifications in Allied Health, Nursing, Aged Care or Mental Health highly desirable

Responsibilities:

ABOUT THE ROLE

A rewarding opportunity for a passionate Dementia professional to join the Dementia Centre team at HammondCare on a Permanent Full-time basis at our Hammondville Site.
The role will provide support to staff at all levels to learn, develop and implement best practice in relation to dementia care by embedding HammondCare’s Model of care and Case management framework. As a Dementia Centre Advisor in the Dementia Centre, you will engage in projects and contribute to ongoing knowledge generation and communication in relation to best practice dementia care and support.

RESPONSIBILITIES

  • Provide dementia specific expert advisory services within HammondCare residential and HammondCare at Home services, through participation in regular clinical review meetings and on the ground support across all cottages for the site allocated.and client homes
  • Build staff capacity through communication, education, mentoring and coaching in practice; this can include spending time with the team on the ground, conducting case study discussions and review of documentations
  • Plan engagement with individuals, families and teams to achieve agreed practice development objectives and quality improvements
  • Work on projects and assignments as required and in line with areas of skill, expertise and knowledge
  • Participate, contribute and learn as part of the Dementia Centre team in team meetings, professional supervision and team activities.
  • Utilise agreed HammondCare and the Dementia Centre processes to communicate, monitor and manage work delegations and projects including evaluation and reporting.
