Description:
An exciting job opportunity has become available at the Newham Diagnostic Memory Clinic which is part of the Newham Community Mental Health Care of Older People.
The team is offering Band 6 post for a 14 months fixed term contract or secondment to cover maternity leave, with a possibility for extension. The opportunity is opened to Registered Mental Health Nurse, Occupational Therapist or Social Worker.
This is a great opportunity for you to enhance your knowledge and skills in memory assessment and dementia care while working with a dynamic and welcoming Multidisciplinary Team.
To provide specialist assessment, care and support to people presenting with cognitive impairment, dementia and co-morbid healthcare needs
Actively engage people with Dementia and Cognitive impairment and their carers in all stages on intervention, ensuring that their views are acknowledged and taken into account
Assess and present individual service user cases in MDT meetings for Team approach to providing care and services to service users and their carers
To laisse with the GPs, Local Authority and other statutory organisations and voluntary agencies to promote partnership working
To act as a care coordinator for a caseload of service users, ensuring that you develop, implement and review the individualised care provided
To develop and facilitate specialist groups within the Team to support those diagnosed with Dementia and their families
To provide post-diagnostic feedback and therapeutic counselling to service users diagnosed with dementia and their families, ensuring that support and appropriate advice are offered to them
To conduct carers assessment and take action where required, including offering of support and referral to other services
To act as a duty officer in line the Team Single Point of Entry (SPE) duty Rota, processing referrals and responding to emergencies as they arise
Take part in the Transformation Process of the Team in line with the NHS Long Term Plan
Ensure that service users receive culturally sensitive care
