Diabetes Lay Educator

You will co-deliver the DESMOND Patient Structured Education Programme to groups of up to 20 adults with Type 2 Diabetes and their carers, family or significant others as set out in the programme manual in a consistent co-ordinated manner. This may be delivered virtually and/or at a variety of community venues. You may be asked to deliver other patient education sessions as appropriate and identified by team leader You will maintain record keeping in line with professional and trust guidance You will become an accredited DESMOND Educator and be responsible for your own clinical professional development and DESMOND accreditation You will participate in mandatory training as required by the Trust You will actively participate in a regular clinical and managerial supervision to support your ongoing professional development You will have the opportunity to contribute to audits and quality improvement projects within the tea
