Description:

Diabetes Nurse (Mental Health) - Band 8a
Permanent - 1 WTE
The post holder will provide expert advanced diabetes clinical care across the Trust’s mental health inpatient sites; Newham, City and Hackney, Tower Hamlets, Luton and Bedfordshire. This includes ordering & reviewing appropriate blood test, reviewing clinical notes, working in partnership with mental health practitioners and patients to review and amend care plans and treatment plans up to prescribing / amending prescribed lifestyle choices and medications.
You will provide strategic vision on the prevention and management of diabetes for adults with mental illness across a number of forums.
Trigger annual diabetic screening.
Facilitate formal diabetes education teaching session for multi-disciplinary team every 4-6 weeks per site.
Have a robust knowledge and understanding of the challenges faced by providing care to this complex and often difficult to engage patient population.
To maintain a clinical caseload, being responsible for admissions, discharges and the implementation of clinical care, including escalation.
Be a credible senior clinician role modeling outstanding evidence based care, sharing knowledge with colleagues.
To be closely involved in establishing and developing interdisciplinary care pathways, protocols and audits.
To contribute to the planning and development of services for this particular group of clients
