Diabetes Specialist Nurses band 6 0.55wte (20.625hrs)
Base; TBC in the Wellingborough / Kettering locality
The Northamptonshire Diabetes Multi – Disciplinary Team (NMDT) promote care closer to home, facilitating a seamless service between acute, community and primary care, avoiding acute hospital admissions by working closely with primary and community care. We focus on enabling people with diabetes to remain healthy by working collaboratively with stakeholders and people with diabetes to promote self-management and avoid acute and long-term complications of diabetes.
The members of this busy and successful Diabetes MDT are looking for Diabetes Specialist Nurse/s to join the team. This is an ideal opportunity to extend skill and gain experience in diabetes care at a specialist level, developing skill in education and increasingly complex and technologically based prescribing decisions.
The team is currently clinically led by a specialist nurse, with nurses, dietitians, support workers, podiatrists and administrative staff, as well as Diabetologist supervision.
DEPENDING ON EXPERIENCE AND SKILL THE POST HOLDER WILL:
Work with secondary care staff to provide assessment, treatment and management of hospital in-patients secure safe discharges from the acute hospital setting.
Work collaboratively with primary and community care, providing specialist input to their caseload management.
Provide a fast access clinic service to reduce acute referrals and hospital admissions.
Take part in the delivery and evaluation of education programmes for patients, social & health care staff.
Non-medical prescriber (desirable but can be achieved in post), facilitate the management of insulin and diabetes related medications and equipment within your case load and your sphere of competence.

The post holder will assist in maintaining a safe, secure and healthy working environment and continuously work towards developing and improving the quality of the service provided.
