Description:

Stockport Foundation Trust is seeking a highly motivated nurse to join their Integrated Diabetes Team to cover maternity leave. We at Stockport take pride with the understanding that the team manage all aspects of the speciality including Inpatient work, pregnancy, insulin pumps, young adults and transition, complex wound and foot alongside community referrals from primary care. The post advertised is a fabulous opportunity for an experienced DSN to apply but we also would like to invite motivated nurses who are currently working within the speciality an opportunity of a development role that would offer a competency framework to optimise your knowledge and skills to become a Diabetes Specialist Nurse of the future.

To assess, plan, prioritise, implement and evaluate nursing interventions to meet the patients’ needs which may be in the community or acute setting.
To actively participate in delivering health care for people with diabetes in Stockport; to ensure high quality and safe standards of care.
To advise, educate and support clients and carers on the management of diabetes by providing specialised programmes of care in relation to health and illness and the development of self-management techniques including individual and group sessions
Adhere to Nursing & Midwifery Council (NMC) Code of Conduct and associated guidelines.
Maintain confidentiality in all aspects of care and employment
Be actively involved in clinical supervision of nursing and midwifery
staff.
Act as a role model at all times.
Maintain relevant professional registration.
Maintain appropriate records adhering to Trust and Nursing &
Midwifery Council (NMC) guidelines on record keeping
