Description:

An exciting opportunity has arisen for a Band 7 Diabetes Specialist Nurse to join the community diabetes team in Coventry.
The Coventry diabetes nursing team provides a service for both in-patients and out-patients within the hospital setting and the community.
The successful candidate will share our commitment to patient centred care. They will be highly experienced, with advanced skills in managing complex diabetes patients in a variety of settings.
They will be able to make autonomous, evidence-based decisions and lead by example to educate and support colleagues in primary and secondary care.
The post holder will work to proactively manage, coordinate and support an identified caseload of patients with diabetes. This will include patient and carer education, annual health checks and oversight of medication management.
The candidate will be an accessible and responsive team member with clinical responsibility for a defined patient group. Able to make informed decisions within their own area and scope of professional practice to ensure agreed standards, objectives and targets are met.
A Non-Medical prescribing qualification is desirable for this post.

Work proactively to manage, coordinate and support an identified caseloads of patients with diabetes. Including patient and carer education, annual health checks and oversight of medication management.
Opportunity to work across digital platforms to support the virtual MDT between the specialist diabetes service and community partners.
To use advanced skills and expert knowledge to identify subtle changes in condition and take appropriate action.
Manage a team and support the daily operational service.
Work collaboratively with In-patient and Out-patient services to deliver excellent patient focused care.
Accountable for own actions in accordance with Code of Professional Conduct. Will contribute towards corporate objectives, acting within local, Trust and statutory guidelines and policies.
Developed communication skills enabling maintenance of high-quality service and ensuring timely management and resolution of unexpected events and situations. Ability to communicate with all members of the multi-professional teams locally.
A knowledgeable team member, supporting staff, service and policy development within a defined area/speciality.
A supportive resource for staff, advising on national, local and Trust policy and procedures/guidelines, ensuring that clinical governance is embedded in practice.
