Responsibilities:
ABOUT THE ROLE
This fixed-term 12-month part-time 5 days/fortnight Dietitian role will join an expanding Restorative Care Team to deliver a high-quality dietetics service to HammondCare clients in the community. This is an exciting role with a broad caseload. Whilst you’ll have the autonomy to structure your own day to meet key deliverables, you will also be supported by an experienced MDT team. Together you will deliver patient-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).
THE ESSENCE OF THIS ROLE
- Provision of a high-quality Dietetics service to HammondCare Restorative Care team, HCP (Home Care Package) and CHSP (Commonwealth Home Support Program) clients including individual 1:1 home visits and group activities at our various sites
- Assess and provide intervention to clients in their own homes, carry out rehabilitation home programs to improve functional independence and quality of life
- Participate as a member of the multidisciplinary team in assessing clients’ level of function and care needs, setting care goals, and providing appropriate treatment and interventions
- Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment
- Participate in quality improvement projects and continuing professional development
- Provide clinical supervision for students
