ABOUT THE OPPORTUNITY

SWSLHD Transitional Aged Care Program (TACP) is seeking a Level 1 / 2 dietitian to support their community and/or residential teams in providing high quality clinical dietetic assessment and intervention to older adults who are accessing the program.
TACP is a unique program focused on the reablement of older adults living in the community. Working with clients, their carer’s, the TACP multidisciplinary team and other relevant stakeholders, you will be contributing to holistic care that ensures better health outcomes and quality of life over a time-limited period post-discharge from hospital.
This position will be based in Fairfield LGA.

Responsibilities:

This position is responsible for providing patient centered and high quality nutrition services to patients in SWSLHD including providing nutritional care to patients as per caseload; provide and participate in education and evaluation; supervise students as appropriate; and participate in ongoing professional development, quality improvement and research as required.
