People Who Care provide person-centred assistance and support to people living with a disability, helping them to achieve their goals and remain as independent as possible. This role will also provide services for our Veterans Program.
Responsibilities:
TO SUCCEED IN THIS ROLE:
- You will need to be caring, patient, responsible and experienced in working with individuals with specific needs. Our clients may be experiencing psychosocial, physical and intellectual challenges.
- You may be asked to support our clients with mobility, personal care, preparation of meals, domestic tasks, accessing their community and other aspects of their daily life.
- Being flexible and proactive in your approach will help you to contribute positively to their wellbeing.
RESPONSIBILITIES:
- Support individuals with a range of activities, such as assisting to maintain their homes (domestic assistance), accessing the community, assistance around the property and attending appointments (social support).
- Support individuals to achieve and meet their goals as specified in their NDIS plan (or Veterans HCP Plan).
- Provide regular case notes and communicate with the Service Coordinator about the health, well-being and progress of the individual.
- Work collaboratively with the individual and the disability services team, and provide support that best meet the individual’s needs.
