Description:

An exciting opportunity has arisen within the Discharge Hub in Bedford and Luton for x3 Band 6 Social Worker, 24 months fixed term(2 years).
The Discharge Hub is a newly developed service designed to support and assess adults who are experiencing a wider range of mental health difficulties to be discharged from In- patient assessment wards, but also all adults who have been admitted into General hospital and need support to be discharged. The Hub is based across Bedford and Luton and may cover some out of area hospitals if there are Bedford and Luton patients who need support for discharge.
The Team will have close connections to the ward working within a rich multi-disciplinary team on the wards and in close partnership with Local Authorities and ICB.
To work as a member of the Discharge Hub representing a social work perspective to all adults experiencing mental health difficulties.
The Discharge Hub Social Worker will be a champion of the social model of intervention within the discharge process from Acute Inpatient Wards and General hospital Wards and will ensure that social factors contributing towards the mental health difficulties are recognised and addressed as far as possible.
Social Workers are expected to work in partnership with service users and carers, identifying and utilising strengths and working closely with other agencies and voluntary groups to ensure good outcomes which, as far as possible, maintain independence. Social Workers will have a good working knowledge of The Care Act, Mental Health Act and Mental Capacity Act/DOLS and other social care legislation. Within the team it is vital that Social Workers maintain a social perspective to their work and promote social care issues within the team.
To offer expert advice and guidance to the team on adult safeguarding issues, including writing reports and representing/supporting conferences and other relevant meetings.
