Responsibilities:
An opportunity exists in the Discharge Support & Liaison team in a permanent, part time position.
Applications are encouraged from Registered Nurses, Occupational Therapists, Physiotherapists, Social Workers, Speech Pathologists, Podiatrists and Dietitians with an interest in helping patient’s access appropriate services to enable timely discharge from hospital.
The Discharge Support & Liaison team is responsible for working collaboratively within the multidisciplinary clinical teams to facilitate patient flow from the admitted inpatient setting through to the most appropriate pathway to prevent readmission and ensure that all patients receive the right care at the right time and in the right place.
