The SEND EHCP Transitions team are seeking to recruit an experienced and dynamic Practitioner to join this new and exciting team!
The role involves working closely with educational settings, families and professionals to provide high-quality targeted support and direct intervention ahead of key transition points for children and young people with Education Health and Care Plans (EHCPs), starting from Key Stage 2, up to Key Stage 4 and above.
You will be passionate about obtaining children and young people’s voice, using creative resources to engage them in conversation around transitions and next steps, including preparation for adulthood. You will be advocating for children and young people, and their families, and ensuring that they are at the centre of planning and reviews carried out by the service.
You will be working as part of a new and exciting team, alongside experienced professionals including Advisors, SENCo’s and Assessment and Planning colleagues to improve outcomes for children and young people.
The EHCP Transitions team works countywide; however, we are looking to recruit in the Hastings area ideally. Your preference for area-based working will be discussed at interview.

We are looking for applicants who:

  • Have experience of supporting children and/or young people with a range of additional needs and those with EHCPs.
  • Can develop and deliver appropriate programmes for children and young adults in conjunction with setting staff
  • Can apply a wide range of approaches to work with children and young adults and their families.
  • Are self-motivated, creative, and have strong organisational skills to effectively manage their workload.
  • Can build positive working relationships with both internal colleagues and external stakeholders.
