Description:

QUALIFICATIONS/EXPERIENCE:

  • Bilingual (English/Spanish) preferred
  • Preferred education: bachelor’s in social science, Psychology, or related field
  • Minimum requirement is one year experience in case management or related social services field.
  • Must maintain a current driver’s license and have personal automobile insurance.

TIME/PHYSICAL REQUIREMENTS

  • Monday-Friday, 8:00am-5:00pm/40 hours per week.
  • Some evening and weekend activities may require flexing schedule.
  • Tasks occur in both office settings and in field; some local and in-state travel may be required (5%)

Responsibilities:

  • Advocates on behalf of individuals to ensure services are delivered in a timely, efficient, accessible manner.
  • Support out-of-care clients to engage in medical care and case management services.
  • Assists in the development and implementation of individual treatment/services plan.
  • Educates individuals on services, resources, and HIV/AIDS information. This includes information on transmission prevention.
  • Identifies and reports unresolved service gaps and barriers and works to implement solutions through telephone contact and face-to-face meetings in accordance with level of need.
  • Informs individuals and their families of their rights as health service consumers.
  • Maintains accurate and timely case management records on clients. This entails documentation regarding demographic, psychosocial, financial, and medical information including daily contact notes.
  • Participates in all required staffing, team meetings and training sessions.
  • Participates in community and inter-agency resource groups addressing issues related to client services.
  • Performs comprehensive psychological needs assessments.
  • Procures, coordinates, and monitors the delivery of needed services in accord with a service plan and identifies levels of need.
  • Provides appropriate referrals and follow-ups according to client needs.
  • Provides crisis intervention as needed. Make appropriate referrals.
  • Completion of all programs specific training within a specified time
