As the Elder Care Manager , you will play a pivotal role in individuals’ lives, making a significant impact through:
- Conducts in-home assessments utilizing program specific tools to identify needs including health, disability and wellness, financial, housing, family, local resources, advocacy and legal
- In collaboration with the client(s) develops an individualized service plan outlining goals and objectives
- Will make and foster connections with community groups, service agencies, and providers to promote coordination and collaboration of resources and services; identify and serve potential clients in the community
Maintains strict adherence with HIPPA regulations and protocolsRegular client visits, in person or by phone as indicated, ensuring safety and all other needs are being meet
Elder Care Manager Minimum Salary: $20.00
Elder Care Manager Maximum Salary: $ 2 1.00
Elder Care Manager Schedule:
Monday-Friday 8:30-4:00 pm
Qualifications:
Bachelor’s Degree in human services or a related field3-5 years of experience dealing with a diverse population
