Description:

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with urological conditions and diagnosis within the surgical decision unit Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within your experience and scope of practice Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Clinical responsibilities will include the following although this is not an exhaustive list as procedures may vary depending upon service needs; History taking and clinical examination. Routine pre-op assessment of patients. Requesting X-rays Patient catheterisation (male and female) Providing cannulation/venepuncture Management of suprapubic catheters Commencing IV fluid Understanding and application of National Early Warning Score (NEWS), escalating concerns to appropriate staff To be able to commence and complete discharge summaries To refer to the wider MDT staff for collaborative care pathways for the patient Working in collaboration with the Multi-Disciplinary teams e.g. Radiology to ensure treatments/procedures are both appropriate and timely To schedule future appointments for patient attendance as required To support ward areas in times of escalation Logically problem solve situations, and seek support as needed Use critical thinking skills as part of the decision making process with regards to patient care, and situational issues that arise Communicate effectively at all times, and escalate issues and concerns timely.
To seek support as needed Creatively support colleagues, and use research proven statistics to provide high standards of care Be aware of trends that can be present within the service or patient groups Have the ability to understand, identify and deal with situations which can cause issues in the future, and demonstrate the appropriate skills and judgement abilities to overcome situational concern To work in partnership with the surgical team Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly Work with patients in order to support compliance with and adherence to prescribed treatments Tackle discrimination and harassment, and promote equality and diversity in the workplace Take responsibility for personal development and education and the development of a Personal Development Plan To enhance the patients experience and to ensure a multidisciplinary approach to the care and management of patients undergoing a range of surgical procedures To have an extended role in supporting the clinical and nursing teams in the management of patient care. Working as part of the surgical service line Support the junior medical roster with the elective care directorate. Having responsibility for patients under the care of the surgical Consultants with supervision from the surgical registrar. Ensuring best practice in infection control the post holder will work closely with staff in work areas.
Ensuring that clinical guidelines are adopted and adhered to, any members of staff found to be non-compliant being dealt with appropriately Improving clinical care standards and treating patients with dignity and respect the post holder will ensure clinical standards are upheld within their own specific area Lead and contribute to the development of policies through audit and ensure their implementation Promote and demonstrate innovative practice and develop own practice as required Ensure that systems are in place to identify and minimise all risks associated with the delivery of patient care with particular reference to clinical risk management and any relevant health and safety legislation Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Encourage others to make realistic self-assessment of their application of knowledge and skills, challenging any complacency or actions that are not in the interest of the public and/or users of services Critically evaluate and review innovations and developments that are relevant to the area of work Take a lead role in planning and implementing changes within the area of care and responsibility. Take the lead in the development of nursing practice guidelines, policies and protocols / care pathways within specialism Demonstrate specialist knowledge of the emergency surgical care patients maintaining knowledge and skills of the highest standard Maintain high profile in the clinical care, act as a role model and provide senior support and advice to staff both internally and externally to the Trust Deliver governance standards in line with the Trust/Governance framework Ensure that competencies of clinical skills and history taking are maintained to deliver quality care Work clinically in the speciality area to deliver training, advice and support to the ward staff. Actively participate in training and education of other staff including nursing, medical and allied health professionals at local and national agendas Provide trust wide education and training on both specialist area of practice and generic nursing and care management Assess, plan, implement and evaluate episodes of patient care in line with Trust Nursing policies Ensure the rights, privacy and dignity of patients, are protected at all times Maintain awareness of local and national nursing agenda Act as a resource for other professionals sharing and developing good practice both within Trust and nationally
