MINIMUM REQUIREMENTS

  • Grade 12 plus Enrolled Nurse qualification
  • HIV and TB management trainings, WBPHCOT will be an added advantage
  • A minimum of 2 years’ experience in HIV and TB programs
  • Registration with the South African Nursing Council as an enrolled nurse

Responsibilities:

PURPOSE OF THE POSITION:

Provides clinical and administrative duties including but not limited to; supervision of case officers, linkage of clients into care, supports case management solutions for adults and pediatrics in attainment of 2nd 95 and 3rd 95. Liaise with various stakeholders, district, health facilities and other government departments.

DUTIES AND RESPONSIBILITIES

  • Blood monitoring and collection as per required monitoring intervals
  • Be familiar with the ROAD TO HEALTH BOOKLET and ensure completion of all relevant section at each visit working closely with P/N
  • Align EPI visits with HIV management of children and adolescents including growth monitoring and development
  • Key entry point screening and testing of children and adolescents 0-19 years for HIV/TB/Covid-19 as per HIV Matrix of Intervention and HIV/TB/PMTCT guidelines, including Antenatal/Post-natal/Breastfeeding clients using IMCI Checklist/PATEST tool/HIV adult screening tool, handholding those tested positive and link them to care
  • Enrol all children and adolescents newly diagnosed with HIV on case management model, for continuity of care and support and facilitate decanting to Differentiated Models of care as per SOP, working closely with community teams and OVC partners
  • Provide preventative counselling for women tested HIV negative during ANC/breastfeeding/PNC and offer Pre-Exposure Prophylaxis (PrEP).
  • Offer index testing services for ANC/breastfeeding/PNC partners and provide HIVSS test kits where necessary
  • Monitor child development and immunisation status of all children <5 years irrespective of HIV exposure history
  • Periodically, conduct MCH clinical file audit, to ensure effective implementation of PMTCT guidelines and effective clinical management of clients.
  • Ensures that all children and adolescents 0-19 years are in optimum user-friendly formularies and regimen as per age & weight, to enhance adherence and VL suppression
  • Take VL blood as scheduled in PMTCT guidelines and enrolled all children and adolescents with VL >50 c/ml on enhanced adherence counselling (EAC), report any issues needing attention to P/N or VL champion
  • Weekly actioning of RFA list as appear on RFA dashboard/ elabs device.
  • When due refer all clients require psychosocial support, disclosure and adherence counselling to social worker via FTL/PN.
  • Facilitate decanting of eligible < 19 years clients and periodically evaluate clients due for script renewal, with assistance of FTL/PN
  • Support establishment of youth-zones and its activities to reach <19 years whom consulted on that youth-friendly corner
  • Work with social workers to establish youth clubs, Post Natal clubs, Viremia clubs and Family care days.
  • Participate in Operation Phuthuma Nerve Centre activities that contributes to MCH indicators, including MCH score card, where applicable
