Responsibilities:
We are seeking an experienced Clinical Care Coordinator, or Senior Registered Nurse with Leadership experience, to join the team in this highly regarded Residential Aged Care Facility in Mid North Coast Region Of NSW.
Key responsibilities include:
- Assesses resident services by following admission procedures, collecting and assessing resident data, and accessing a range of evidence sources.
- Plans resident services by developing care plans, determining priorities of care,establishing realistic and measurable goals, and utilising available support and service resources.
- Documentation to support continuity of care planning such as recording observations, documenting in progress notes, and completing other documentation such as accident incident reports.
- Coordinate and actively participate in the implementation of assessments and care planning for new residents (including respite residents) upon admission to the Facility.
- Ensure resident assessments, initial care plans/reviews and other relevant clinical documentation are completed accurately and reflect the complete individualised needs of the resident and legislative requirements.
- Works with the ACFI manager to drive completion of ACFI assessments by the care team.
- Foster a supportive team environment and encourage professional development.
- Adhere to legislation and common law that impact nursing practice.
- Provide services in line with funding guidelines for project initiatives.
To be successful in the role you will have:
- Bachelor of Nursing
- Current AHPRA registration
- Aged care management experience or Senior RN experience
- Strong written and verbal communication skills
- National Police clearance
- Flu vaccination and Covid vaccinations
