JOB DESCRIPTION SUMMARY

Reporting to the Leader, Inner City Youth Program and receiving day-to-day supervision from the Occupational Therapist, the Mental Health Rehabilitation Assistant provides a variety of physical, emotional, social, and advocacy supports, facilitates learning of life skills and community integration activities to clients in the program in a community health setting. Works collaboratively with the interprofessional team members in the development of individualized recovery plans; monitors the functioning levels of the client through the application of current principles of recovery-based mental health and addiction care and general knowledge of psychiatric disorders; assesses potential emergency situations and makes necessary interventions as required; and establishes, maintains and enhances therapeutic relationships based on respect. Assists in maintaining a safe environment for clients and the team. Participates in team meetings, engages in team planning and quality improvement activities, and maintains and updates own knowledge.
Qualifications / Skills and Education:
Education
Completion of a Rehabilitation Assistant Program, plus one year of recent, related experience or an equivalent combination of education, training and experience.
Skills and Abilities
Ability to communicate effectively both verbally and in writing.
Ability to deal with others effectively.
Physical ability to carry out the duties of the position.

Ability to organize work.
Ability to operate related equipment.
Duties and Responsibilities:
In consultation with interprofessional team members, prepares, carries out group and evaluates individual activities to meet the clients’ physical, cognitive and psycho-social needs. Activities may include: exercise, self-care training, life skills training, current events, community integration and other therapeutic activities. Demonstrates and instructs the techniques of related activities and communicates activity plans with other disciplines. Reports back to therapist(s) or other appropriate team member any changes in clients’ condition.
In consultation with interprofessional team members, plans and conducts group interventions using a variety of therapeutic approaches to meet clients’ physical, social, cultural and psychological needs.
Coordinates, prepares and carries out client outings. Ensures the safety and well being of clients in their home and community environments.
Documents first-hand observations regarding clients’ status by recording observations on client record, flow sheet forms and/or incident reports as required. Reports observations concerning clients’ status to designated therapist or appropriate team member.
Cleans and maintains equipment and reports malfunctioning equipment or equipment needs to designated therapist. Assists the designated therapist in maintaining equipment by carrying out minor adjustments and identifying and reporting operational problems.
Maintains established inventory lists, levels of supplies and equipment and replaces stock in accordance with program procedures.
Provides direction to volunteers during activity programs, and demonstrates related client specific techniques colleagues and students as required.
Fabricates adaptive devices under the direction of the designated therapist by cutting out material, sewing items, assembling parts and/or making alterations as required.
Assists in fostering a pleasant, stimulating and inviting environment, through
encouraging client participation and activity, such as providing seasonally relevant activities.
Records workload statistics or other related information as required.
Participates in staff meetings, team conferences, in-service education programs and presentations, and attends related courses.
Performs other related duties as assigned
