Description:

Central Cheshire Integrated Care Partnership (CCICP) is expanding its team of General Nursing Assistants (GNAs) who work as part of the Integrated Placement of Care Hub. GNAs provide care and support to patients at home. Examples include assisting with personal care, medication administration, working with a patient in the preparation of light meals, assistance with catheters or stomas, and support with rehabilitation exercises.
CCICP is looking for people with experience of working in care, or as a Health Care Assistant, with a passion for providing high quality care to patients.
GNAs work across CCICP’s five care community hubs: Nantwich, Northwich, Winsford, Crewe & SMASH (Sandbach, Middlewich, Alsager, Scholar Green and Haslington).
GNAs can expect to work shifts of 8am - 4pm and/or 2pm - 10pm, across 7 days a week.

As a General Nursing Assistant (GNA), you will work as part of the Integrated Placement of Care Hub (IPOCH). The service focuses on a ‘home first’ approach; one of rehabilitation and promotion of independence for patients leaving acute/bed-based settings or requiring step up care in their own homes.
A significant part of the role also involves supporting end of life patients in their own homes, providing a dignified and quality approach to care for the patients and their families at a challenging time.
You will be working under the direction and with full support from the wider care community teams, including district nurses, therapy teams and advanced community practitioners.
The role is varied and full training will be provided by your own dedicated GNA trainer. They will help you to develop and enhance your skill set. A band 6 nurse will also act as your mentor, supporting you personally with appraisals and one-to-one meetings.
You will also have the day-to-day support of our senior GNA practitioners. The team is dedicated to supporting your development and ensuring you have all the skills you need to enjoy your role and provide the very best care.
The successful candidate will undertake record keeping using an electronic device for which training will be provided.

To work under the direct supervision and guidance of the nursing team.
Acts on own initiative in delivering patient care in the community.
To support multi-disciplinary health professionals in providing a comprehensive rehabilitation programme to prevent avoidable hospital admission.
To provide care and rehabilitation for service users within their own homes under the clinical direction of qualified staff. This includes assessing patient’s condition through observation and clinical skills, recording appropriately on EMIS and ensuring the relevant professionals are informed in a timely manner.
Communicate factual information to patients/carers that requires patience, persuasion and reassurance.
To provide high quality care for patients and their families within the community setting; including the provision of end of life care.
Occasional/frequent distressing or emotional circumstances.
Careful use of equipment/handles patients personal possessions; ensure equipment used by patients e.g. hoist is safe and properly used. To maintain accurate personal and professional records adhering to trust policies and procedures. T
To support patients with administration of medications
To ensure the most efficient and cost-effective utilisation of all resources (human physical and financial).
To report accidents/incidents of patients or self which occur whilst on duty and to complete appropriate forms immediately
